2. Applicants for the free sample offer will receive one LENTICULE Disc.

3. Offer is limited to one sample per customer/organisation department.

4. There may be a delivery charge depending on the location of the delivery address. Applicants will be notified in advance if a delivery charge applies.

5. For delivery to the USA a CDC import permit is required. For delivery to Canada an import permit from Health Canada is required.

6. Applicants must submit their request for a free sample using the form located on the HPA Culture Collections website at: www.hpacultures.org.uk/products/lenticulediscs/freelenticulediscsample.jsp

7. The HPA Culture Collections reserves the right to withdraw this promotional offer in the event of over subscription.
